The Lake Stevens Police Department, located at 2211 Grade Road, is an acceptance agent for the United States Passport Services.

Passport services are available Monday-Friday between 1:00 p.m. and 4:30 p.m. No appointment required, walk-ins only

All applicants must be present at time of application. Applicants younger than age 16 must be present and accompanied by BOTH parents. Applicants who are 16 or 17 years old must be present and accompanied by ONE parent, both having sufficient ID on them.   

Applicants must bring a check or money order with the DS-11 application to cover the passport fee only. Two separate payments will need to be made at the time of application.

A U.S. Passport is an extremely important document. Extra precautions are taken to ensure the integrity of the passport application process.

Passport fraud is a felony, punishable by up to 10 years in jail and / or a $250,000 fine.

What Are the Current Passport Regulations & How Do They Affect Me?

 As of January 23, 2007, all persons, including U.S. citizens, traveling by air between the United States and Canada, Mexico, Central and South America, the Caribbean, and Bermuda are required to present a valid passport, Air NEXUS card, U.S. Coast Guard Merchant Mariner Document, or an Alien Registration Card, Form I-551, if applicable. 

The passport requirement does not apply to U.S. citizens traveling to or returning directly from a U.S. territory, as U.S. citizens returning directly from a U.S. territory are not considered to have left the United States and do not need to present a passport. U.S. territories include: Guam, Puerto Rico, the U.S. Virgin Islands, American Samoa, Swains Island, and the Commonwealth of the Northern Mariana Islands.

What Documents Do I Need to Apply for a Passport?

Proof of Citizenship
  • Certified U.S. birth certificate; or
  • Previous fully valid U.S. Passport and copy of birth certificate if under 16 when passport obtained;
  • Report of Birth Abroad (Form FS-240); or
  • Certification of Birth Abroad (Form DS-1350); or
  • Certificate of Citizenship or Naturalization from BCIS
Proof of Identity
  • Driver's license; or
  • Government issued ID
Passport Photographs
  • One photograph, 2 x 2 square, with a white or off-white background
  • Photographs must meet U.S. Passport standards
  • You may either bring acceptable photos, or you may have your passport photos taken here for $16 when you apply for your passport

What if I Do Not Have a State-Certified Certificate?

You may obtain a state-certified birth certificate by contacting the city or town where you were born, or you may order one online (or call 1 (800) 255-2414).

How Much Does It Cost to Get a Passport?

    • Those 16 and 17 must be accompanied by one parent
  • $80 Passport Book and/or $15 Passport Card for Persons younger than age 16

Two separate payments must always be made:
  • The first payment which covers the Application Fee and Expedite Fee (if applicable), can only be paid by check or money order made payable to U.S. Department of State.
  • The second payment, which consists of the Execution Fee ($35 per applicant), photos ($16 per applicant, if taken here), and postage fee (for expedite service only) may be paid by debit or credit card, cash, money order or check made out to City of Lake Stevens.

How Long Does It Take to Get a Passport?

Due to heavy demand, passports will be received 6-8 weeks following application.
